The process is begun by the patient or family member completing a risk assessment request – an on-line application [ Click Here ] . 


After the low-dose CT scan is reviewed by a radiologist the patient risk is reported.  If the risk is  low or intermediate the patient is directed to the appropriate support contact.


If the results of the CT scan is evaluated as High Risk after the radiologist review, the  user is given the option to sign up for follow-up through a secure portal.  The user  chooses an imaging center,  and imaging center contacts user to schedule.


After the initial screening CT, the patient and/or patient’sphysician are notified of the results and are also provided recommendation for follow up.


The final screening step is the patient scheduled for follow up.  A reminder will be provided at the appropriate
